Medical Heritage Summary
With a Medical Heritage Summary, we will compile a summary of information only for medical history. Our summary will include information that was provided at the time of the adoptive placement, as well as any medical updates that have been reported since the time of adoption. Search Due to Medical Necessity
The Code of Virginia allows adoptees or adoptive parents to request updated medical history from the birth family if there is documented medical urgency to obtain this information. This service requires the adoptee or adoptive parents to provide supporting documentation to initiate the search. Once this information is obtained, we will search to locate the birth parents and request their current medical history. The fee for this service is $75. A heritage summary is a compilation of the adoption record containing medical and background information about an adoptee’s birth family. A heritage summary does not contain copies of documents from the adoption record and it will not include identifying information about the birth family. Although the information in each heritage summary is unique, in most instances they include information about the birth parents’ ages, physical appearance, religious beliefs, race, culture and/or ethnic backgrounds, education, employment history, and hobbies or interests. They may also include information about birth parents’ extended family members, a description of the relationship between the birth parents, and the circumstances surrounding the adoption.
