Adoption Services Referral Form
Through the state funded Adoptions through Collaborative Partnerships (ATCP) grant, CHSVA offers our DSS partners a range of adoption services including finalization assistance (the preparation of reports including the Full Disclosure, Report of Investigation & Adoption Addendum) and child-specific recruitment. Children must be 6 or older or a part of a sibling group and must have TPR. Special cases can be approved at VDSS discretion. Click here for an Adoption Services Referral form.
